You can use this form to be placed on our waiting list for 2012 and future seasons.  But it is hard to predict how many openings there will be and our waiting list is getting very long so you might want to also check out other CSA farms that may have space for 2011.  Public/OtherCSAs lists some and you can search on your zipcode at http://localharvest.org/.
